Immunizations Records
 
  • Download 2020-21 Immunization Parent Letter from the CDPHE
  • Immunization records are a required document we must have on file at the school.
  • Students must be up to date on all shots or a waiver signed if you have chosen not to vaccinate your student.
  • Students will not be able to come to school until we receive the immunization records or wavier starting in the new school year.
  • We will contact all families that are not up to date on their immunizations.
  • Current 5th-grade students that will be in 6th grade the following year need to get the TDAP shot over summer break and we need the updated record

  • Descargue la Carta de Vacunación para padres 2020-21 del CDPHE
  • Los registros de vacunación son un documento obligatorio que debemos tener en los archivos de la escuela.
  • Los estudiantes deben estar al día con todas las vacunas o entregar una exención firmada si ha elegido no vacunar a su estudiante.
  • Los estudiantes no podrán asistir a la escuela hasta que recibamos los registros de vacunación o la carta de exención al inicio del nuevo año escolar.
  • Nos comunicaremos con todas las familias que no estén al día con sus vacunas.
  • Los estudiantes actuales de 5to grado que estarán en 6to grado el siguiente año escolar necesitan recibir la vacuna TDAP durante las vacaciones de verano y necesitamos el registro de vacunación actualizado.
